Senior & Disabled Services provides Information & Referral
(I&R) to people who need help finding resources and services. I&R
can be accessed by phone, by email, and in person. I&R endeavors
to link the person seeking help to the right resource, whether it is
provided directly by Senior & Disabled Services or by another agency

What services and programs are available from Senior & Disabled
Services?
Senior & Disabled serves clients age 60 and over and disabled
individuals age 18 and over. Senior & Disabled Services offers assistance
with Information & Referral
Service, Supplemental Nutrition Assistance Program (SNAP), Senior
Meals Program, Medicaid
(sometimes called Oregon Health Plan), help paying for
Medicare premiums, In-Home Care
Services,
assistance with Medicare Part D Choice Counseling,
and help paying for long term care.

Senior & Disabled
Services has three full-service and three community satellite offices.
In addition, S&DS operates twelve Senior Meals

What services are available from S&DS’ Senior Connections
Program, and how can I contact one of the Program’s Area Coordinators?
Senior Connections links older adults (60+ years) with the resources
and services they need in order to continue to live independently in
their own homes. The Program’s Area Coordinators and volunteers
provide a variety of services to seniors and unpaid family caregivers,
including information and referral, screening and certification for various
benefits (such as, low income energy assistance, Meals on Wheels, medical
transportation, and respite for unpaid caregivers), and case management
for older people who need this level of assistance but do not qualify
for Medicaid-funded case management help. For more information, including

Beyond Senior & Disabled Services, are there other agencies that
provide help to people with disabilities and senior citizens?
Yes, many
other public and not-for-profit agencies and organizations provide help
within Lane County to people with disabilities and senior citizens. For
